Through LSI’s Daily Supported Community Living, adult Iowans with disabilities have the 24-hour care and support they need to thrive. Each day is spent working on their personal goals and taking one more step toward an independent lifestyle.

Eligibility
- Must be 18 or older with one of the following waivers:
- Intellectual Disability (ID) Waiver
- Home-Based Habilitation
- Brain Injury (BI) Waiver

Contact your case manager or care provider to discuss LSI!
Are you new to the process?
Contact your local Department of Human Services office to get started. They can help you find a case manager or care coordinator, apply for a waiver, and refer you to LSI for services.

Email your completed form to ReferSCL@LSIowa.org! Upon receipt of your referral, an LSI supervisor will contact you with more information about next steps.
