You want the best for your loved one. We believe you deserve time to take care of yourself, as well! LSI provides Respite Care to children and adults across the state, which allows caregivers to take a much-needed break.

Must have one of the following waivers:
• Children’s Mental Health (CMH) Waiver
• Intellectual Disability (ID) Waiver
• Home-Based Habilitation
• Brain Injury (BI) Waiver
• Health and Disability (HD) Waiver

First Circle Friends

If you’re a caregiver for a loved one experiencing memory loss, you want them to enjoy a full, meaningful life. At First Circle Friends, we help to make that happen. While your loved one enjoys cognitively stimulating activities in a friendly, safe environment, you have time to take a deserved break and care for yourself, as well.

Eligibility

First Circle Friends is available for residents of the Clinton and Des Moines areas.

Participants are experiencing early to mid-stage memory loss, such as Alzheimer’s or a brain injury.

While trained volunteers are available for assistance, participants should be able to feed themselves, use the bathroom independently, and take any needed medications before or after a session.

Are you new to the process? Contact your local Department of Human Services office to get started. They can help you find a case manager or care coordinator, apply for a waiver, and refer you to LSI for services.

Complete this form and provide a social history, current plan, and assessment with your referral. Upon receipt of your referral, an LSI supervisor will contact you with more information about next steps.
